According to Art. 13 of Regulation (EU) 2016/679 of the European Parliament and of the Council of April 27, 2016. on the protection of individuals with regard to the processing of personal data and on the free movement of such data and the repeal of Directive 95/46/EC (the “General Data Protection Regulation”) (“RODO”), we kindly inform you that:

  2. The Administrator has appointed a Data Protection Officer. The Data Protection Officer is responsible for all matters related to the Administrator’s processing of personal data. The Administrator processes your personal data in the form of voice sound and the data transmitted using it in order to enhance security and improve the quality of service to the Administrator’s patients and contractors, pursuant to Art. 6 paragraph. 1(a) RODO (the data subject has consented to the processing of his/her personal data for one or more specified purposes), which consent is given by continuing the phone call. Consent to recording is voluntary, but is necessary in order to obtain a phone call;
  4. Call recording includes all incoming calls to the following numbers used by the Administrator: 22 690 03 33, +48 535 386 208;
  5. call recordings are stored by the Administrator depending on the size of the recorded data, i.e. to overwrite the data, for no longer than a period of three (3) months from the time of the recording, or for the period necessary to complete proceedings under the law or proceedings in which the recordings can or will be used as evidence. After the expiration of these periods, recordings containing personal data shall be destroyed;
  6. The administrator secures and makes personal data available only to the relevant authorities authorized under the relevant laws, such as the police, the prosecutor’s office, the court. Beyond

  7. The Administrator shall keep your personal data confidential and secure them from unauthorized access by third parties in accordance with the principles set forth in applicable regulations;
  8. Your personal data will not be transferred to a third country (outside the European Economic Area) or to an international organization;
  9. Your personal data will not be processed by automated means and will not be profiled;

In accordance with the principles set forth in the data protection legislation and to the extent provided therein, the data subject has the right to: access personal data concerning him/her, rectify (amend) them, complete them if they are incomplete, delete or restrict processing, transfer and receive a copy, object to processing, or withdraw consent to the processing of personal data (if processing is based on consent; withdrawal of consent does not affect the lawfulness of processing carried out before withdrawal);
  11. The data subject is entitled to lodge a complaint with the Office for Personal Data Protection against the Controller if he/she believes that the processing of personal data violates the law;
