The basic indications for the hyaluronidase treatment are primarily complications of the previously administered hyaluronic acid or the dissolution of a poorly selected filler. The treatment is also recommended for patients who struggle with problems such as threatening necrosis of the skin and subcutaneous tissue, caused by the administration of hyaluronic acid, visual disturbances, violent allergic reaction, occurrence of cocci, asymmetry, biofilm formation, occurrence of organism intolerance to the previously administered hyaluronic acid, undesirable effect of filler displacement, excessive amount of hyaluronic acid filler, maintaining swelling due to water retention at the place of filler application, the appearance of lumps and unevenness caused by a earlier, and shallow administration of hyaluronic acid.

The application of the enzyme hyaluronidase is not always possible. The contraindication is, inter alia, allergy to insect venom or bovine and sheep proteins, damage or ulceration of the skin surface in the treated areas, fungi, viral and bacterial infections. If the patient is at risk of skin and / or subcutaneous tissue necrosis or other very serious complication, the only contraindication is the refusal to consent to the medical intervention.

In the case of the planned administration of hyaluronidase, another contraindication is also the unrealistic expectation of the results that can be obtained, arterial hypertension, autoimmune diseases, pregnancy and breastfeeding, or active cancer.

The procedure

The treatment does not require any preparation – you should go to the doctor without makeup, and he will disinfect the area where the enzyme will be applied, immediately before the injection. Before the application of hyaluronidase, consult a doctor who will perform a detailed medical history and ensure the proper course of the procedure.

In cases where there are any indications for the application of the enzyme (impending skin necrosis), hyaluronidase is administered without conducting an allergic test. A small amount of hyaluronidase is administered to the forearm area. The site is then monitored for an adverse allergic reaction. If there is no disturbing phenomenon, the hyaluronic acid will be dissolved after 15-30 minutes.

The enzyme is applied by using a fine needle. The injection takes place in the area where the filler was previously injected – lips, nose area, etc. The substance is administered in a small amount, directly to the papule formed after the administration of hyaluronic acid. Occasionally, small swellings or bruises appear after injecting the enzyme, but they disappear very quickly. Some of the patients react to hyaluronidase with a local inflammatory skin reaction, urticaria.

The first 6 hours after the procedure, it is strictly prohibited from touching the areas where the injection of the enzyme was concerned. 72 hours from its performance, you should not massage, rub or scratch the treated areas of the body. The full effect becomes visible just after 72 hours. Hyaluronidase dissolves hyaluronic acid as a filler and the skin’s own acid. The whole process takes about 3 weeks, after which the cavity is gradually rebuilt. The improvement in the skin condition is usually visible from 1 to 7 days after the treatment. The body excretes and metabolizes the enzyme within one week of the application of hyaluronidase.

One treatment is usually enough to obtain satisfactory results. 72 hours after the application of the enzyme, the doctor assesses the obtained results. He can then decide whether to reapply hyaluronidase.

Administration of hyaluronidase is not a threat to the life and health of the patient, provided that it is carried out by a specialist. Administration of the enzyme is associated with a certain risk of adverse reactions (just like any interference in the body). The form that is most life-threatening is an allergic reaction. That is why it is so important that the procedure is performed by qualified doctors.
